GLP-1 Microdosing Membership Agreement

Please review the membership terms below before enrolling in a semaglutide or tirzepatide microdosing plan.

1. Cancel Anytime +
Cancel anytime. To avoid being billed for the next month, cancellation must be submitted before your upcoming billing date.
2. Initial Visit Is Non-Refundable +
The initial telehealth visit fee is non-refundable and covers clinical evaluation, program planning, and dosing recommendations.
3. Medication Already Ordered or Shipped Is Non-Refundable +
Once medication has been ordered, processed, dispensed, or shipped, it cannot be refunded. If medication has shipped for that cycle, that month’s fee is non-refundable.
4. Monthly Subscription Billing +
Your payment method will be charged automatically each month through Square. Payment failures will be automatically reattempted.
5. No Refunds for Partially Used Months +
If you cancel in the middle of a billing cycle, your membership remains active until the end of that cycle. No prorated refunds are provided.
6. Medication Refill Schedule +
Medication is included in your membership and refilled approximately every 90 days. Tide Wellness coordinates and manages refill scheduling.
7. Clinical Messaging & Support +
Secure messaging is included for dosing adjustments, GI symptom management, and microdose-related questions. Some concerns may require a telehealth check-in.
8. Clinical Oversight & Safety +
You agree to report significant symptoms or changes in health. Tide Wellness may modify dosing or schedule additional evaluation for safety.
